Job Description

Our Client is seeking Licensed Social Workers to provide services in the Chicagoland area. 

 

SUMMARY OF THIS OPPORTUNITY:

• Screen, assess, diagnose, create treatment plans, and conduct therapeutic interventions with clients
• Maintain regular, ongoing contact with clients, caregivers, and physicians.
• Conduct reassessments and monitor clients' progress.
• Complete and submit timely paperwork and reports.
• Review and modify care plans as directed by the client/caregiver during the reassessment and obtain the client's signature of approval.
• Maintain regular, ongoing contact with clients, caregivers, physicians, and service providers listed on the service plan through home visits and phone calls.

REQUIRED EXPERIENCE AND EDUCATION FOR LICENSED SOCIAL WORKER:

• Must have a Professional Educator License (PEL) in the state of Illinois with an endorsement in Social Work.
