Job Details Provide diabetes self-management education and training to individuals and groups. Oversee planning, implementation, and evaluation of our DSME/T program. Develop and direct program marketing, curriculum selection, and materials review. Ensure DSME/T accreditation requirements are met and maintained. Establish and maintain partnerships with community groups, payers, and referral sources. Requirements: Certified as a diabetes education specialist with a minimum of 3 years of experience. 1000 hours of practical experience and completion of a national certification exam. Knowledgeable in chronic disease management, program management, and educational processes. Proficient in computer skills, including Word, Excel, PowerPoint, and email.
